2026 SFLOA Registration

 

We are excited to have you join our family of Stripes. This Registration is to join the South Florida Lacrosse Officials Association (SFLOA). This bring you one step closer to officiating Boys High School, Middle School, Tournaments, and/or Youth Games. 

Final approval of this application for FHSAA Games depends on completion of: 

1) Verification of current FHSAA Registration.

2) Completion of this registration form and payment of SFLOA dues.

3) Completion of the "Jessica Lunsford Act" requirements and submission of an active Clearance Badge.


  • Includes year of Full SFLOA Membership & Gift of a new 2" stripe FHSAA Shirt ($49.99 value). Necessary to officiate High School and Middle School Games.

  • Includes year of Junior SFLOA Membership. Necessary to officiate Youth League Games.

By signing below, I acknowledge and agree that information provided here is accurate and true. I also acknowledge and understand that approval of my application WILL NOT constitute an employment agreement between myself and the FHSAA, the County School Board(s), Private School(s), or their Athletic Association(s), nor the SFLOA. Rather, I will act as, and be considered to be, an independent contractor while serving as an FHSAA registered official. I also agree to be bound and abide by all of the FHSAA’s and SFLOA’s rules and regulations governing the conduct of contest officials at FHSAA sanctioned events and elsewhere. I also acknowledge that it is my responsibility to read and understand the Requirements and Guidelines for SFLOA Officials.
